Case Manger - NonProfit experience - (Housing or Shelter experience)

Responsibilities:
Provide case management services including assessments, service planning, home/office visits, and crisis intervention. Assist clients in accessing medical care, housing, employment programs, and supportive services to promote independent living. Maintain documentation, collaborate with service providers, and ensure compliance with program guidelines.
Qualifications:
Bachelor’s in Human Services
Housing (supportive services)
Experience with HIV/AIDS, mental health, substance abuse, or homelessness populations
Strong communication and documentation skills
Bilingual (English/Spanish) a plus
Proficiency with Microsoft Office
Details:
Full-time, 35-hour work week, non-remote (office and field work).
