LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Apple Valley, California.

This is an acute care case management RN role focused on discharge planning, utilization review, and care coordination . Candidates must have 2+ years of recent acute care case management experience , including experience in high-volume hospital environments.The RN will manage a caseload of up to 25 patients , coordinate with physical therapy and social services, and ensure safe and timely discharges. Strong working knowledge of HMO/IPAs, DME arrangements, community placement, and services for patients with complex social needs (e.g., homelessness, substance abuse, mental health) is required. Must be competent with EPIC and able to arrange ambulance transport and SNF referrals independently.
Candidates must also be comfortable organizing family meetings, handling prior authorizations, and addressing CMS compliance and discharge barriers.
Weekend rotation required (minimum 4 weekend shifts per 6-week period ). Must be a seasoned traveler ; first-time travelers will not be accepted .
