Job Description

Healthcare Staffing Professionals has an immediate need for a Community Health Worker to work in San Diego to support the Aging and Independence Services Health Promotion Unit. The Aging & Independence Services Health Promotion Unit is seeking Community Health Workers (CHWs) to assist with the design, implementation, and coordination of CalFresh Healthy Living (CFHL) and Chronic Disease Self-Management Education (CDSME) programs.
CFHL aims to empower low-income older adults to lead healthy and active lifestyles by providing evidence-based nutrition education programs, followed by “Community Change” sessions. The Community Change sessions engage participants in creating policy, systems, and environmental changes that aim to improve access to healthy food and physical activity opportunities. CFHL services also include the production of a quarterly nutrition-focused newsletter, as well as tabling at various community events.

Topics covered include fall prevention, mental health, physical activity, medication management, nutrition, and more. Participants learn skills and techniques for the daily management of their health, including exercise, healthy eating, symptom management (pain, fatigue, sleep, shortness of breath, stress, and depression), weight loss, and communication skills. Core self-management skills taught include action planning, problem solving and decision making. AIS is licensed to offer three curricula (chronic disease, chronic pain, and diabetes) in English and Spanish.
CFHL and CDSME programs are delivered in a small group format (10-20 participants) at community locations such as senior centers, churches, and libraries; housing sites; and online. AIS staff provide quality assurance, technical assistance, and logistical support for both programs.
